Rates:​

• Psychological Testing: $400 per hour

• Neuropsychological Testing: $400 per hour

Insurance:

​Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?

• What is my deductible, and has it been met?

• What is the coverage amount?

• Is a referral from my primary care physician required?

• Does insurance require pre-authorization for testing?

We currently accept the following insurance plans for assessments and testing only:

• Cigna

• United Behavioral Health

• Aetna

• Anthem Blue Cross Blue Shield

• Kaiser

• Tricare

• Medicare

• Medicaid

Please get in touch with us to check whether we accept your insurance. Under special circumstances, we can work with some insurance plans to obtain a single-agreement plan if we are not in network with your insurance.

 

While we are contracted with most medical insurance providers, some health insurance policies can have carve-out plans for mental health benefits. These carve-out plans are usually managed by a different insurance company than the medical health insurance.  Unfortunately, we may not be contracted with some of these carve-out programs even though we are contracted with the health plan. While we will try to assist with verifying your coverage, you are responsible for ensuring that your insurance will cover the services provided in this office.  Please call your insurance company for verification.

All balances from deductibles, co-pays, co-insurance, and non-covered services are your responsibility and are expected to be paid.

Reduced Fee and Sliding Scale Services:

Reduced fee services are available on a limited basis.

Payments:

Cash, check, and all major credit cards are accepted for payment.

Cancellation Policy:
If you do not show up for your scheduled appointment and have not notified us within 72 business hours in advance, your account will be assessed the cost of an hour fee ($400 for an assessment). You will be required to pay the fee in full prior to scheduling another session. Please understand that by not providing us with advanced notice of a session cancellation, we are unable to fill that spot.
